The World Health Organization (WHO) has declared this outbreak a global health emergency, with over 300 suspected cases and 88 deaths reported so far. The situation is dire, as an official from the Rwampara health zone, speaking anonymously, revealed to Al Jazeera that "people are dying here every day." Rwampara, the epicenter, has seen an average of five deaths daily over the past three days.
In Bunia, the capital of Ituri Province, fear and uncertainty prevail. Residents like Constant Ulimwengu and Sherif Musa express their shock and concern over the resurgence of Ebola amidst an ongoing humanitarian and security crisis. The initial suspected case was a nurse who passed away at the Bunia Evangelical Medical Centre, and the subsequent funeral, conducted without proper Ebola-prevention measures, highlights the challenges in containing the outbreak.
Community Mistrust and Challenges
Elie Badjo, an analyst on universal health coverage in the DRC, warns that community mistrust could hinder containment efforts. People are leaving comments suggesting that powerful entities may be taking advantage of the situation for research purposes, or that there is a risk of extermination due to the ongoing war in the east. This mistrust poses a significant challenge to the authorities' efforts to raise awareness and implement preventive measures.
The densely populated province of Ituri, affected by violence and controlled by armed groups, makes containing the outbreak even more difficult. Mining regions, trading hubs, and constant population movement further increase the risk of rapid disease spread. Jean-Jacques Tamfum Muyembe, a Congolese professor and virologist, who co-discovered Ebola 50 years ago, warns that the risk of this outbreak spreading is greater than what he witnessed in Kikwit in 1976.
A Fragile Healthcare System
The DRC's healthcare system is already fragile, as evidenced by the previous Ebola outbreak from 2018 to 2020, which killed nearly 2,300 people in the eastern provinces of North Kivu and Ituri. Delayed responses, insufficient healthcare infrastructure, and community mistrust were key lessons from that outbreak. The new outbreak could further strain this fragile system.
Unsafe burials, delayed reporting, and limited access to healthcare contribute to the risk of widespread transmission. Priorities include coordinating community outreach, increasing testing and treatment capacity, and protecting healthcare workers. However, the region's social and economic pressures, including population movement, mining operations, and local trading activities, make surveillance and containment challenging. Armed groups further complicate response efforts by limiting access to certain areas.

Regional cooperation is vital to prevent the spread to neighboring provinces and countries.
